On your N16 item:
It is fully appropriate (and necessary) to assign *domains* to SC items. Realms can then bind them to code systems as desired/appropriate.  The intent isn't necessarily that all of them would be locally-defined.  Things like the Address parts will likely have formally defined code tables associated with them, and may be enforced across all messages in a realm.

Attached is a spread sheet and word document that detail all of the Harmonization items intended for discussion on Friday, February 21, 2003 M&M Conference Call.  With the exception of a single item, all of the information is in the spreadsheet.  The codes for the Annotated ECG message are more complex and are discussed in detail in the Word document.
